人们常将“小三”译为"the third party"，虽然也有人这么讲，但是以美国一般人来说，如果在没有交代事情背景的情况下，突然对他说"Sarah is the third party."，那么他会无法理解的。即使交代了事情背景，他们也不一定能听懂这个说法是什么意思。

“小三”这一称呼大多指女性，我们可以用"misstress", "lady friend"等来表达“外头的女人”，例如：Who is his mistress?

"The other woman/man"也是“第三者”的常用表达法，例如：Sarah is the other woman who caused the breakup of their marriage.
