【急】asp如何文件上传

我在dw里弄了个文件选择框，提交后竟然没有文件！！！
怎么才能实现文件上传啊

下面是用ASP写的，已经测试过
文件上传有个非常重要的地方，就是限制格式，严格规定只有什么格式才可以上传
否则可能会被黑客利用
这些光用DW自动生成是做不出来的 要自己写代码
