我记得没有 you'd better to do 这种用法, you'd better do 是固定用法. 但是为什么词典中的例句有很多类似的用法呢?
如:
It would be better to go for a walk than stay at home all alone. 与其坐在家里发闷,莫如出去走走。
I think you had better to go to see a doctor. 我看你最好还是去看医生。
是因为后面的 go for a walk 和 go to see a doctor 都是词组的原因吗?

这句话是正确的,这里并不是"had better",只是普通的形容词"be better"的用法。

这句话是错误的,或者说是不严谨,但是不妨碍理解。
"you had better"后面接原形不定式,而不是to不定式的原因,是它们出现的时期不同。
"you had better do"原本的形式是"you were better do",出现在公元900年前后,那时候to不定式尚未发达,只有原形不定式。
"you were better do"转变成"you had better do"是在公元1400年以后,那时候to不定式的使用变得频繁,但仍然是与原形不定式平分秋色的状态。
之后到近代英语期,to不定式逐渐取代原形不定式变为主流,但直到现代仍然有部分语句使用的是原形不定式而不是to不定式,"you had better do"便是其中之一。
很明显在语义上,to不定式和原形不定式是可以互相置换的,但是实际使用哪个,需要遵循约定俗成。
